Why do we collect and disclose personal information?
LPI collects and discloses personal information only for the purpose of serving the insurance related needs of our customers. This will enable us to underwrite, bind or renew an insurance policy for you and ensure that your information is accurate and up-to-date. It will assist us in assessing your ongoing needs and to be able to offer you other related products or services.

What personal information do we collect?
The information we can collect will vary depending on the type of policy you are seeking. The information would include, but not be limited to such basic information as name, address and phone numbers, email address, birth dates and marital status. We might also collect claims and credit history, driving record, medical information, and details about your property and employment history.

Who do we collect from and disclose your personal information to?
We will be collecting from and disclosing information to many third parties in the course of our duties. We would communicate with you, insurance companies, other insurance brokers or agents, credit organizations, financial institutions, medical professionals, motor vehicle/driving licensing authorities and other professionals working with us such as adjusters or lawyers. In the event that LPI should be involved in a business transaction such as a sale or merger or other type of amalgamation, your personal information may be shared with such prospective parties so as to complete the business transaction.

How do you provide us with your consent?
We accept any of the following as your consent for us to use your personal information now or in the future:
-Your receipt of this Personal Information Policy, unless you advise LPI of the withdrawal of your consent.
-Your express consent or acknowledgement contained within a written, verbal or electronic application process.
-Your voluntary provision of personal information for the purpose of acquiring an insurance contract or related service or product. This includes all information previously provided to LPI.
-We may determine that by seeking insurance through our firm that your consent has been implied to allow us to deal with your personal information in a reasonable manner.
-Your consent as provided by your authorized representative such as a legal guardian or power of attorney.

Subject to certain legal or contractual restrictions and reasonable notice, you may withdraw your consent at any time. In some cases, refusing to provide or withdrawing your consent to use certain personal information may mean we cannot provide you with insurance coverage, other requested products, services or information.

Collection, retention, safeguards and accuracy of your personal information – LPI will only collect, use, disclose or keep your personal information that is necessary for the identified purpose or as required by law. We will maintain your information in as accurate and complete a form as possible. LPI will protect your personal information by security safeguards appropriate to the sensitivity of the information. Safeguards will vary depending on the format, location and storage of the information. We will obtain the appropriate consent from you for the collection, use and disclosure of your personal information.

Accountability and Access – LPI has a designated privacy officer who is accountable for the security and maintenance of your records and may be contacted at the below number if you:
-Wish to access or change your consent to the ways in which we collect, use or disclose your information.
-Have a complaint about our privacy policies or the manner in which we have handled your personal information.
-Have any questions about our privacy policies or wish to request additional information.
